Participating Insurance Companies

Patients are typically referred to Nephrology Physicians by their primary care physician. Our office staff will work closely with your doctor's office to ensure that we have the most accurate information on your current medical condition, including the results of any recent medical tests. If required by your insurance carrier, we will request that your primary care physician secure the appropriate prior authorization from your insurance company. 

After we receive the information from your physician our Nurse will review your records to expedite your appointment. Once the Nurse has reviewed your record our New Patient Referrals Coordinator will contact you upon receiving the information from your doctor's office.


You may access our New Patient Packet to give you the opportunity to complete the required forms in advance and bring them to your first appointment. Our Coordinator will also identify a convenient time and schedule your initial visit with one of our nephrologists.

Please bring the following information to your first appointment with our nephrologist:

  • All of your prescription bottles, including any over-the-counter medications. Or you can click on the Medication list and print it out for your appointment. The medication list is a fillable PDF. 
  • All of your current insurance coverage ID cards
  • All completed forms from the New Patient Packet. You are more than welcome to fill out the package at the time of your visit.

You can refer yourself directly to Yuma Nephrology, P.C. for Kidney or Hypertension Care. Please complete the Referral Form for Consultation and provide the documents that are listed on the form.
